What it is and how it works
Clopidogrel is a prodrug whose active metabolite irreversibly blocks the platelet P2Y12 ADP receptor; aspirin irreversibly acetylates platelet COX-1, inhibiting thromboxane A2 formation. Together they provide additive inhibition of platelet aggregation.
Indications
Prevention of atherothrombotic events in adults already taking both clopidogrel and aspirin, e.g. after acute coronary syndrome or percutaneous coronary intervention with stenting.
Dosage and administration (summary)
One tablet (clopidogrel 75 mg with aspirin 75–100 mg) orally once daily, as continuation of separately initiated therapy.
Contraindications
Active pathological bleeding such as peptic ulcer or intracranial haemorrhage; severe hepatic impairment; hypersensitivity to NSAIDs or aspirin-induced asthma; third trimester of pregnancy.
Warnings and precautions
Increased bleeding risk; caution with anticoagulants, NSAIDs and SSRIs and before surgery. Reduced effect in CYP2C19 poor metabolisers and with omeprazole or esomeprazole. Thrombotic thrombocytopenic purpura has been reported rarely.
Pregnancy
Avoid, and contraindicated in the third trimester because of the aspirin component.
