Long-acting recombinant follicle-stimulating hormone agonist (gonadotrophin)

What it is and how it works

A recombinant glycoprotein consisting of the FSH alpha subunit and a beta subunit extended with the C-terminal peptide of hCG, giving a prolonged half-life. A single injection provides sustained FSH activity that initiates and maintains multifollicular development for the first 7 days of ovarian stimulation.

Indications

Controlled ovarian stimulation in combination with a GnRH antagonist for the development of multiple follicles in women participating in assisted reproductive technology programmes; in some markets also induction of spermatogenesis in adolescent males with hypogonadotropic hypogonadism (with hCG).

Dosage and administration (summary)

Women: a single subcutaneous injection on stimulation day 1, typically 100 micrograms for body weight 60 kg or less and 150 micrograms for body weight above 60 kg, under the supervision of a fertility specialist; daily FSH may follow from day 8 if required.

Contraindications

Tumours of the ovary, breast, uterus, pituitary or hypothalamus; abnormal vaginal bleeding without known cause; primary ovarian failure, ovarian cysts or enlarged ovaries; history of ovarian hyperstimulation syndrome (OHSS) or a previous cycle with more than 30 follicles; polycystic ovarian syndrome.

Warnings and precautions

Ovarian hyperstimulation syndrome can occur and patients should be monitored by ultrasound. Multiple pregnancy, ectopic pregnancy and thromboembolic events are increased with assisted reproduction. Use is not recommended with a GnRH agonist protocol.

Pregnancy

Not indicated during pregnancy.
