Dapoxetine

What it is and how it works

Dapoxetine inhibits neuronal serotonin reuptake, potentiating serotonergic activity at pre- and post-synaptic receptors involved in the central control of the ejaculatory reflex. Its rapid absorption and elimination make it suitable for on-demand use.

Indications

Treatment of premature ejaculation in men aged 18–64 years who meet defined diagnostic criteria.

Dosage and administration (summary)

30 mg orally taken as needed approximately 1–3 hours before sexual activity, with a full glass of water; may be increased to 60 mg if tolerated. Maximum one dose every 24 hours.

Contraindications

Significant cardiac conditions (heart failure, conduction abnormalities, significant ischaemic or valvular heart disease), history of mania or severe depression, moderate to severe hepatic impairment; concomitant MAOIs, thioridazine, SSRIs, SNRIs, tricyclic antidepressants or other serotonergic drugs, and potent CYP3A4 inhibitors.

Warnings and precautions

Orthostatic hypotension and syncope can occur, particularly at the start of treatment; patients should sit or lie down if prodromal symptoms occur and avoid driving if affected. Avoid alcohol and recreational drugs. Use with caution in epilepsy, bleeding disorders and in patients with risk of raised intra-ocular pressure.
