What it is and how it works

Dihydrotachysterol is a synthetic reduced analogue of vitamin D2 that is hydroxylated in the liver to its active form without requiring renal 1-alpha-hydroxylation. It increases intestinal calcium absorption and mobilises calcium from bone, raising plasma calcium.

Indications

Hypocalcaemia due to hypoparathyroidism (idiopathic or post-operative) and pseudohypoparathyroidism; some forms of vitamin D-resistant rickets and renal osteodystrophy.

Dosage and administration (summary)

Dosing is individualised by the specialist according to plasma calcium, with higher initial doses followed by a lower maintenance dose; frequent monitoring of plasma calcium is essential.

Contraindications

Hypercalcaemia, vitamin D toxicity and hypersensitivity to the substance.

Warnings and precautions

Has a narrow therapeutic margin and a prolonged action, so hypercalcaemia may persist for weeks after overdose; monitor plasma calcium regularly, especially at the start of treatment and after dose changes. Use with caution in renal impairment, nephrolithiasis and with digoxin or thiazide diuretics.

Pregnancy

Use only when clearly needed with careful calcium monitoring; excessive doses may harm the fetus.

Storage and supply

Store below 25 °C, protected from light.
