What it is and how it works

Edrophonium is a quaternary ammonium compound that reversibly inhibits acetylcholinesterase, increasing acetylcholine at the neuromuscular junction and at muscarinic sites. Its action is rapid in onset (within about 1 minute) and brief (about 5–10 minutes).

Indications

Diagnosis of myasthenia gravis (Tensilon test) and differentiation of myasthenic from cholinergic crisis; reversal of non-depolarising neuromuscular blockade in some settings.

Dosage and administration (summary)

Diagnostic test in adults: typically 2 mg intravenously, followed after 30–45 seconds by a further 8 mg if there is no adverse reaction; an anticholinergic (atropine) should be immediately available.

Contraindications

Mechanical intestinal or urinary obstruction and hypersensitivity to edrophonium.

Warnings and precautions

Can cause severe bradycardia, cardiac arrest, bronchospasm and increased secretions; use with great caution in asthma, cardiac arrhythmias, recent myocardial infarction, hypotension and epilepsy. Resuscitation equipment and atropine must be available during testing. Because of its brief action it is not used for maintenance therapy.

Pregnancy

Use only if clearly needed; data are limited.

Storage and supply

Store below 25 °C, protected from light.
