Eprosartan

What it is and how it works

Eprosartan selectively blocks the binding of angiotensin II to AT1 receptors, inhibiting angiotensin II-mediated vasoconstriction and aldosterone release and thereby lowering blood pressure.

Indications

Treatment of essential hypertension.

Dosage and administration (summary)

Adults: 600 mg orally once daily; may be increased to 800 mg daily if needed, with a lower starting dose sometimes used in the elderly or in hepatic or renal impairment.

Contraindications

Pregnancy (second and third trimesters, and generally avoided throughout), severe hepatic impairment, haemodynamically significant bilateral renal artery stenosis, and concomitant aliskiren in diabetes or moderate to severe renal impairment.

Warnings and precautions

Can cause hyperkalaemia and deterioration of renal function, particularly with renal artery stenosis, heart failure, potassium supplements or potassium-sparing diuretics; monitor potassium and creatinine. Symptomatic hypotension may occur in volume- or sodium-depleted patients. Dual blockade of the renin–angiotensin system is not recommended.

Pregnancy

Contraindicated in pregnancy because of fetal renal toxicity and oligohydramnios; switch to an alternative when pregnancy is planned or diagnosed.
