- Cough preparation; opioid antitussive with expectorant (product category)
What it is and how it works
These syrups combine codeine, an opioid that suppresses the cough reflex centrally in the medulla, with expectorant ingredients (such as guaifenesin, ammonium chloride or sodium citrate) and sometimes an antihistamine. Codeine is partly converted to morphine by CYP2D6, which contributes to both its effect and its toxicity; composition varies by product.
Indications
Short-term symptomatic relief of troublesome cough in adults when other measures have failed.
Contraindications
Children under 12 years, patients known to be CYP2D6 ultra-rapid metabolisers, breastfeeding, respiratory depression, acute asthma, chronic obstructive airway disease with respiratory failure, and risk of paralytic ileus.
Warnings and precautions
Codeine can cause respiratory depression, sedation, constipation and dependence; concomitant use with benzodiazepines, alcohol or other CNS depressants increases the risk of profound sedation and respiratory depression. Use should be short-term, and suppressing a productive cough may be undesirable; it is subject to controlled-drug and misuse precautions.
Pregnancy
Avoid, particularly near term, because of the risk of neonatal respiratory depression and withdrawal.
Storage and supply
Store below 25 °C, protected from light.
