Antidiabetic: premixed human insulin (soluble insulin with isophane insulin)

What it is and how it works

A fixed mixture of soluble (regular) human insulin and isophane (NPH) human insulin, commonly 30/70. The soluble fraction provides onset within about 30 minutes, while the isophane fraction gives intermediate-acting basal cover; insulin lowers blood glucose by promoting peripheral glucose uptake and inhibiting hepatic glucose production and lipolysis.

Indications

Treatment of diabetes mellitus requiring insulin.

Dosage and administration (summary)

Subcutaneous injection usually once or twice daily, about 30 minutes before a meal; dose individualised according to blood glucose monitoring. Resuspend by gentle rolling and inversion before use; not for intravenous use.

Contraindications

Hypoglycaemia; hypersensitivity to human insulin or excipients.

Warnings and precautions

Hypoglycaemia is the main risk, especially with missed meals, exercise, alcohol or renal impairment. Rotate injection sites to avoid lipodystrophy and cutaneous amyloidosis; transfer between insulin types or brands requires medical supervision. Combination with thiazolidinediones may cause fluid retention and heart failure.

Pregnancy

Insulin is the treatment of choice in pregnancy; requirements change through pregnancy and need close monitoring.
