- Anthelmintic (imidazothiazole) with immunomodulatory properties
What it is and how it works
Levamisole is a nicotinic acetylcholine receptor agonist in nematode muscle, causing spastic paralysis of the worms, which are then expelled in the faeces. It also has immunomodulatory effects, restoring depressed T-cell and phagocyte function.
Indications
Ascariasis and hookworm infections; it has also been used as an immunomodulator, for example in steroid-sensitive nephrotic syndrome in children.
Dosage and administration (summary)
Ascariasis: a single oral dose of 150 mg in adults (2.5 mg/kg); hookworm may require repeat dosing. Immunomodulatory regimens are individualised by the specialist.
Contraindications
Hypersensitivity to levamisole; pre-existing blood dyscrasias.
Warnings and precautions
Prolonged or repeated use may cause agranulocytosis, which can be fatal; monitor blood counts during long-term use and stop at the first signs of infection. Vasculitis, encephalopathy and rash have been reported. A disulfiram-like reaction may occur with alcohol, and it may potentiate warfarin and phenytoin.
Pregnancy
Avoid in pregnancy, especially the first trimester.
Storage and supply
Store below 25 °C, protected from light.
