What it is and how it works
Pentixather is a cyclic pentapeptide ligand of the chemokine receptor CXCR4, which is overexpressed in many haematological malignancies. Labelled with lutetium-177, it delivers beta radiation to CXCR4-expressing tumour cells and to the bone marrow, producing marked cytotoxic and myeloablative effects.
Indications
Investigational CXCR4-directed endoradiotherapy in advanced haematological malignancies such as multiple myeloma, lymphoma and acute leukaemia, usually as part of conditioning before haematopoietic stem cell transplantation, after confirmation of CXCR4 expression by PET imaging; not an established licensed indication.
Dosage and administration (summary)
Administered by intravenous infusion in specialised centres; activity is determined individually according to dosimetry and the treatment protocol.
Warnings and precautions
Causes profound myelosuppression, so stem-cell support must be available; renal and hepatic toxicity and tumour lysis may occur. Radiation-protection precautions are required for patients, carers and staff.
Pregnancy
Contraindicated in pregnancy, as with other therapeutic radiopharmaceuticals.
Storage and supply
Store in lead shielding according to the manufacturer’s instructions and radiation-protection regulations; short shelf life due to radioactive decay.
