What it is and how it works
Nalidixic acid inhibits bacterial DNA gyrase (topoisomerase II), blocking DNA replication in susceptible Gram-negative organisms; it achieves therapeutic concentrations mainly in urine.
Indications
Uncomplicated lower urinary-tract infections caused by susceptible Gram-negative bacteria (now largely superseded because of resistance).

Contraindications
History of convulsions, porphyria, infants under 3 months and hypersensitivity to quinolones.
Warnings and precautions
May cause CNS effects including raised intracranial pressure and convulsions, photosensitivity and haemolysis in G6PD deficiency. Quinolone-class risks such as tendinitis and tendon rupture apply. Use with caution in hepatic or renal impairment; it may enhance the effect of warfarin.
Pregnancy
Generally avoided in pregnancy because of arthropathy seen in animal studies.
