- Pharmacologic class
- Vitamin B3 (niacin); lipid-regulating agent
What it is and how it works
Nicotinic acid is a precursor of NAD and NADP. In pharmacological doses it inhibits lipolysis in adipose tissue and hepatic VLDL synthesis, lowering LDL-cholesterol and triglycerides and raising HDL-cholesterol.
Indications
Prevention and treatment of niacin deficiency (pellagra); dyslipidaemia as an adjunct to diet when other agents are unsuitable (no longer recommended routinely as add-on to statins).
Dosage and administration (summary)
Oral, with food; lipid-lowering doses are started low and increased gradually to reduce flushing, while much lower doses are used for nutritional deficiency.
Contraindications
Active liver disease or unexplained persistent rise in transaminases, active peptic ulcer and arterial bleeding.
Warnings and precautions
Flushing and pruritus are common; aspirin taken beforehand may reduce flushing. May cause hepatotoxicity (especially with sustained-release forms), hyperglycaemia, hyperuricaemia and gout; monitor liver function, blood glucose and uric acid. Combination with statins may increase the risk of myopathy.
Pregnancy
Nutritional doses are acceptable; lipid-lowering doses should be avoided.
Storage and supply
Store below 25 °C, protected from moisture.
