What it is and how it works
Piperazine acts as an agonist at GABA receptors on nematode muscle, causing hyperpolarisation and flaccid paralysis of the worms, which are then expelled alive by normal peristalsis.
Indications
Ascariasis (roundworm) and enterobiasis (threadworm/pinworm) infections; now largely replaced by benzimidazoles such as mebendazole and albendazole.
Dosage and administration (summary)
Oral, weight-based: ascariasis usually a single dose (about 75 mg/kg, maximum about 3.5–4 g in adults), repeated after 2 weeks if needed; enterobiasis daily doses for 7 days, repeated after a week if needed.
Contraindications
Epilepsy or history of seizures, severe hepatic or renal impairment, and hypersensitivity to piperazine.
Warnings and precautions
Neurotoxic effects such as dizziness, ataxia, incoordination (‘worm wobble’), visual disturbances and seizures may occur, especially with overdose or renal impairment. Gastrointestinal upset and hypersensitivity reactions may occur. Treat all household members in threadworm infection and reinforce hygiene measures.
Pregnancy
Avoid in the first trimester; benzimidazoles or other agents are generally preferred.

Store below 25 °C, protected from light.
