What it is and how it works
Cryo-poor plasma is the supernatant remaining after removal of cryoprecipitate from fresh frozen plasma. It is depleted of fibrinogen, factor VIII, factor XIII, fibronectin and particularly the high-molecular-weight von Willebrand factor multimers, but retains albumin, other coagulation factors and ADAMTS13.
Indications
Replacement fluid in therapeutic plasma exchange for thrombotic thrombocytopenic purpura (as an alternative to fresh frozen plasma); also used as a source plasma for fractionation.
Dosage and administration (summary)
Dosing is individualised according to the plasma exchange protocol and patient’s plasma volume, as determined by the haematologist or transfusion specialist.
Contraindications
Not suitable for correction of fibrinogen, factor VIII or von Willebrand factor deficiency; IgA deficiency with anti-IgA antibodies; use as a volume expander.
Warnings and precautions
Must be ABO-compatible with the recipient. Risks include allergic and anaphylactic reactions, transfusion-associated circulatory overload, transfusion-related acute lung injury and transmission of infection. Use only through a blood administration set with a filter, after thawing at a controlled temperature.
Pregnancy
May be used in pregnancy when clinically indicated.
Storage and supply
Store frozen at −25 °C or colder; once thawed, use promptly according to national blood transfusion guidelines.
