What it is and how it works
Oral poliovirus vaccines contain live attenuated poliovirus (novel OPV type 2 is a genetically stabilised type 2 strain with reduced risk of reversion to neurovirulence) and induce intestinal and humoral immunity. Inactivated poliovirus vaccine (IPV) for injection contains killed poliovirus types 1, 2 and 3 and induces serum neutralising antibodies.
Indications
Prevention of poliomyelitis as part of routine childhood immunisation; novel OPV type 2 is used under WHO guidance for outbreak response to circulating vaccine-derived poliovirus type 2.
Dosage and administration (summary)
Oral vaccine is given as drops (usually 2 drops per dose) and IPV by intramuscular injection (0.5 mL), according to the national immunisation schedule or outbreak-response campaign protocols.
Contraindications
Hypersensitivity to any vaccine component; oral live vaccine should not be given to severely immunocompromised individuals or their household contacts where IPV is advised.
Warnings and precautions
Live oral vaccines carry a very rare risk of vaccine-associated paralytic poliomyelitis and of vaccine-derived virus circulation; postpone vaccination in acute febrile illness, and for oral vaccine, in severe diarrhoea or vomiting.
Pregnancy
IPV may be given in pregnancy if there is a substantial risk of exposure; live oral vaccine is generally avoided unless the risk of exposure is high.
Storage and supply
IPV: store at 2–8 °C and do not freeze. Oral vaccine: store frozen (around −20 °C) for long-term storage, or at 2–8 °C for the limited period stated by the manufacturer; follow vaccine vial monitor guidance.
