What it is and how it works
Bromide ions are thought to hyperpolarise neuronal membranes by passing through chloride channels (including GABA-A receptor channels), reducing neuronal excitability. It has a very long elimination half-life (around 12 days), so steady state is reached slowly.
Indications
Severe, drug-resistant epilepsies, particularly generalised tonic-clonic seizures in childhood epilepsy syndromes (e.g. Dravet syndrome), usually as add-on therapy.
Dosage and administration (summary)
Dosing is individualised by the specialist, titrated gradually and guided by clinical response and serum bromide concentrations.
Contraindications
Hypersensitivity to bromides; severe renal impairment; pregnancy and breast-feeding.
Warnings and precautions
Bromism (sedation, ataxia, confusion, psychiatric symptoms) and acneiform skin eruptions may occur; sodium chloride intake strongly influences bromide elimination, so dietary salt should be kept constant. Serum bromide may cause falsely raised chloride results in some laboratory assays.
Pregnancy
Should not be used in pregnancy.
Storage and supply
Store below 25 °C.
