What it is and how it works
Pralidoxime binds to organophosphate-inhibited acetylcholinesterase and removes the phosphoryl group, restoring enzyme activity, mainly at nicotinic sites (reversing muscle weakness and paralysis). It is effective only before ‘ageing’ of the inhibited enzyme and is used together with atropine, which controls muscarinic effects.
Indications
Adjunct to atropine in the treatment of poisoning by organophosphorus insecticides and nerve agents.
Dosage and administration (summary)
Given by slow intravenous injection or infusion as early as possible; a commonly used regimen (WHO) is a loading dose of about 30 mg/kg followed by a continuous infusion of about 8 mg/kg/hour, adjusted to clinical response.
Contraindications
Hypersensitivity to pralidoxime; the iodide salt should be avoided in patients with iodine hypersensitivity.
Warnings and precautions
Rapid injection may cause tachycardia, laryngospasm, muscle rigidity and hypertension; reduce dose in renal impairment. It is of little value in carbamate poisoning, and the iodide content may affect thyroid function with prolonged use.
Pregnancy
Should be used in pregnancy when needed for life-threatening poisoning.
