What it is and how it works
Human rabies immunoglobulin is prepared from plasma of donors immunised against rabies and contains high titres of neutralising antibody. It provides immediate passive immunity, neutralising virus at the wound site until active immunity from vaccination develops.
Indications
Post-exposure prophylaxis, together with rabies vaccine, in previously unvaccinated persons after category III exposure (or category II in immunocompromised persons) to a suspected rabid animal.
Dosage and administration (summary)
Given once at the start of post-exposure prophylaxis (day 0, or up to day 7 after the first vaccine dose) at 20 IU/kg; as much as anatomically feasible is infiltrated into and around the wounds, without exceeding the calculated dose.
Contraindications
Not to be given to persons with documented complete pre- or post-exposure vaccination; hypersensitivity to human immunoglobulins (relative, given the fatal nature of rabies).
Warnings and precautions
Do not exceed the recommended dose, as it may suppress the response to vaccine; never inject in the same syringe or site as the vaccine. Use with caution in IgA deficiency and in thrombocytopenia (intramuscular injection); it may interfere with live viral vaccines for several months.
Pregnancy
May be used in pregnancy, as rabies is fatal and post-exposure prophylaxis is not contraindicated.
Storage and supply
Store at 2–8 °C; do not freeze; protect from light.
