- Therapeutic radiopharmaceutical (phosphorus-32 sodium phosphate)
What it is and how it works
Sodium phosphate labelled with phosphorus-32, a pure beta emitter with a half-life of about 14.3 days, is incorporated into rapidly proliferating cells and bone mineral in the same way as stable phosphate. The beta radiation suppresses haematopoietic cell proliferation in the bone marrow and irradiates skeletal metastatic sites.
Indications
Polycythaemia vera and essential thrombocythaemia (mainly in elderly patients unsuitable for other therapy); palliation of pain from skeletal metastases.
Dosage and administration (summary)
Dosing is individualised according to protocol by nuclear medicine or haematology specialists; given orally (or intravenously for some products) as a single dose, repeated if required after haematological assessment.
Contraindications
Pregnancy and breast-feeding, severe bone marrow suppression (e.g. leucopenia or thrombocytopenia), and patients in whom long-term leukaemogenic risk is unacceptable (e.g. younger patients).
Warnings and precautions
Causes bone marrow suppression; monitor blood counts regularly. It carries an increased long-term risk of acute leukaemia, and radiation-protection measures apply to body fluids after administration.
Pregnancy
Therapeutic radiopharmaceuticals are contraindicated in pregnancy; pregnancy must be excluded before administration.
Storage and supply
Store in appropriate lead shielding in accordance with national regulations for radioactive materials and the manufacturer’s instructions; use within the stated shelf-life after calibration.
