Sodium Picosulfate

What it is and how it works

Sodium picosulfate is a prodrug hydrolysed by colonic bacteria to the active diphenol, which stimulates colonic mucosa to increase peristalsis and promotes water and electrolyte accumulation in the colon; the effect usually begins 6–12 hours after an oral dose.

Indications

Short-term treatment of constipation; bowel evacuation before radiological, endoscopic or surgical procedures (often in combination with magnesium salts).

Dosage and administration (summary)

Oral, preferably at bedtime: adults usually 5–10 mg once daily for constipation; bowel preparation regimens follow the specific product instructions.

Contraindications

Ileus or intestinal obstruction, acute abdominal conditions such as appendicitis, acute inflammatory bowel disease, severe dehydration and severe abdominal pain with nausea and vomiting.

Warnings and precautions

Prolonged use may cause electrolyte imbalance (especially hypokalaemia) and dependence on laxatives; use for the shortest time necessary. Adequate fluid intake is required, particularly for bowel preparation. Caution in the elderly and in patients taking diuretics or corticosteroids.

Pregnancy

Generally considered acceptable for short-term use when dietary measures and bulk-forming laxatives are ineffective.
