What it is and how it works
Spectinomycin binds to the 30S ribosomal subunit and inhibits bacterial protein synthesis; it is mainly bacteriostatic. Its clinical use is essentially limited to Neisseria gonorrhoeae.
Indications
Uncomplicated anogenital gonorrhoea, particularly when cephalosporins cannot be used (e.g. allergy) or in resistant infection.
Dosage and administration (summary)
Given as a single deep intramuscular injection of 2 g (up to 4 g, divided between two sites, in some situations).
Contraindications
Hypersensitivity to spectinomycin.
Warnings and precautions
Not reliably effective for pharyngeal gonorrhoea and not active against syphilis or chlamydia, so co-treatment and follow-up testing are needed. Injection-site pain, nausea, dizziness and hypersensitivity reactions may occur.
Pregnancy
May be used in pregnancy when cephalosporins are unsuitable, according to treatment guidelines.
Storage and supply
Store below 25 °C; use the reconstituted suspension promptly.
