What it is and how it works
TRODAT-1 is a tropane (cocaine analogue) derivative labelled with technetium-99m that crosses the blood–brain barrier and binds to presynaptic dopamine transporters in the striatum. Reduced striatal uptake on SPECT reflects loss of nigrostriatal dopaminergic neurons.
Indications
Brain SPECT imaging of the dopamine transporter to support the diagnosis of Parkinson’s disease and other presynaptic parkinsonian syndromes and to distinguish them from essential tremor or drug-induced parkinsonism.
Dosage and administration (summary)
Activity is individualised by the nuclear medicine specialist according to the manufacturer’s instructions and local protocols.
Contraindications
Hypersensitivity to the kit components.
Warnings and precautions
Drugs that bind to the dopamine transporter (e.g. cocaine, amfetamines, bupropion, methylphenidate, some antidepressants) may reduce uptake and should be reviewed before the study. The scan does not differentiate Parkinson’s disease from other presynaptic degenerative parkinsonian syndromes. Radiation exposure should be justified and minimised; interrupt breast-feeding temporarily.
Pregnancy
Use only when clearly necessary, weighing diagnostic benefit against fetal radiation exposure.
Storage and supply
Store the kit as specified by the manufacturer; use the labelled product within the stated time, in lead shielding.
