Tetracosactide Acetate
Injection, Suspension · Parenteral · 1 mg/1mL
- List code
- 1205
- Pharmacologic class
- Synthetic corticotropin analogue (ACTH 1–24)
What it is and how it works
Tetracosactide consists of the first 24 amino acids of natural corticotropin and has its full biological activity. It stimulates the adrenal cortex to produce cortisol and other corticosteroids; the plasma cortisol response after a standard dose is used to assess adrenocortical function. A depot (zinc-complexed) formulation provides prolonged stimulation for therapeutic use.
Indications
Diagnostic testing of adrenocortical function (short and, with depot formulation, long Synacthen tests) in suspected adrenal insufficiency; depot formulation occasionally used therapeutically where corticotropin therapy is indicated.
Dosage and administration (summary)
Short test: 250 micrograms by intramuscular or intravenous injection, with plasma cortisol measured before and 30 (and/or 60) minutes after injection. Depot formulation is given intramuscularly in doses and intervals determined by the specialist.
Contraindications
Allergic disorders, particularly asthma, because of the risk of anaphylaxis; the depot formulation is contraindicated in neonates (contains benzyl alcohol) and must not be given intravenously.
Warnings and precautions
Hypersensitivity reactions, including anaphylaxis, may occur; administer where resuscitation facilities are available and observe the patient for at least 30 minutes. Prolonged therapeutic use can cause corticosteroid-type adverse effects (fluid retention, hypertension, hyperglycaemia, infection risk). Interpretation of the test may be affected by oestrogens or recent corticosteroid therapy.
Pregnancy
Use for diagnostic purposes only if clearly necessary; therapeutic use in pregnancy is not recommended.
Storage and supply
Store refrigerated at 2–8 °C, protected from light.
