List code

Pharmacologic class
Diagnostic radiopharmaceutical (201Tl thallous chloride); myocardial perfusion agent

What it is and how it works

Thallium-201 is a cyclotron-produced radionuclide (physical half-life about 73 hours) that emits mercury X-rays and gamma photons. As a potassium analogue it is actively taken up by viable myocardial cells via Na+/K+-ATPase in proportion to regional blood flow, then redistributes over several hours, allowing differentiation of ischaemic from infarcted myocardium.

Indications

Myocardial perfusion scintigraphy at stress and rest (including redistribution and viability studies) in coronary artery disease; parathyroid and some tumour imaging in selected centres.

Dosage and administration (summary)

Activity and imaging protocol are individualised by the nuclear medicine specialist according to local protocols.

Contraindications

Hypersensitivity to the product; contraindications to the stress agent or exercise testing also apply.

Warnings and precautions

Stress testing requires resuscitation facilities. Radiation dose is higher than with technetium-based agents, so the examination must be clearly justified. Breast-feeding should be interrupted for a prolonged period after administration.

Pregnancy

Use only when clearly necessary, as thallium crosses the placenta and the radiation dose is relatively high.

Storage and supply

Store below 25 °C in lead shielding; use before the expiry time stated on the label.
