What it is and how it works
Platelet concentrates are prepared from pooled whole-blood donations (buffy coat or platelet-rich plasma method) or collected by apheresis from a single donor. Transfused platelets adhere to damaged endothelium, aggregate and support haemostasis, temporarily raising the platelet count in patients with thrombocytopenia or platelet dysfunction.
Indications
Prevention and treatment of bleeding in patients with severe thrombocytopenia or functional platelet disorders, e.g. bone marrow failure, chemotherapy, massive transfusion, cardiac surgery, and before invasive procedures below accepted platelet thresholds.
Dosage and administration (summary)
Dosing is individualised by the treating specialist and transfusion service according to platelet count, clinical situation and national transfusion guidelines; an adult therapeutic dose is usually one apheresis unit or one pooled unit.
Contraindications
Generally avoided in thrombotic thrombocytopenic purpura and heparin-induced thrombocytopenia except for life-threatening bleeding.
Warnings and precautions
Platelets are stored at room temperature and carry the highest risk of bacterial contamination of all blood components; febrile, allergic reactions, transfusion-related acute lung injury and circulatory overload may occur. ABO/RhD compatibility should be respected where possible, and anti-D immunoglobulin considered for RhD-negative females of childbearing potential receiving RhD-positive platelets. Use leucodepleted products and irradiated components for patients at risk of transfusion-associated graft-versus-host disease.
Pregnancy
May be given in pregnancy when clinically indicated, with attention to RhD compatibility.
Storage and supply
Store at 20–24 °C with continuous gentle agitation; do not refrigerate; shelf-life is short (usually 5–7 days, according to the blood service).
