What it is and how it works
Vitamin B12 is a cofactor for methionine synthase and methylmalonyl-CoA mutase, and is required for DNA synthesis, red-cell maturation and maintenance of myelin in the nervous system.
Indications
Treatment and prevention of vitamin B12 deficiency, including pernicious anaemia, deficiency after gastrectomy or ileal resection, malabsorption and dietary deficiency; megaloblastic anaemia due to B12 deficiency.
Dosage and administration (summary)
Intramuscular (or deep subcutaneous): an initial course of 1 mg (1000 micrograms) several times weekly to replenish stores, then maintenance 1 mg every 1–3 months depending on the salt and the cause; regimens with neurological involvement are more intensive. Oral high-dose B12 may be used in dietary deficiency. Dosing follows national guidance.
Contraindications
Hypersensitivity to cobalamin or to any excipient (including cobalt).
Warnings and precautions
Confirm the diagnosis before treatment, because B12 can mask folate deficiency and folic acid alone can mask B12 deficiency while neurological damage progresses. Monitor potassium early in treatment of severe megaloblastic anaemia (hypokalaemia). Rarely, hypersensitivity reactions occur after injection.
Pregnancy
Required in pregnancy; treatment of deficiency should not be withheld.
