What it is and how it works
Leucocyte-reduced FFP is fresh frozen plasma from which white cells have been removed by filtration, preserving coagulation factors and other plasma proteins while reducing leucocyte-associated risks. Transfusion replaces deficient coagulation factors.
Indications
As for fresh frozen plasma: multiple coagulation factor deficiencies with bleeding or before invasive procedures, massive transfusion, and plasma exchange in thrombotic thrombocytopenic purpura, particularly where leucocyte reduction is required by policy.
Dosage and administration (summary)
Dosing is individualised by the specialist according to coagulation tests, clinical response and transfusion protocol; ABO-compatible plasma should be used.
Contraindications
Not indicated for volume expansion alone or for correction of abnormal coagulation tests without bleeding or planned procedures; caution in IgA deficiency with anti-IgA antibodies.
Warnings and precautions
Leucocyte reduction lowers the risk of febrile non-haemolytic reactions, HLA alloimmunisation and leucocyte-associated virus transmission (e.g. CMV), but allergic reactions, anaphylaxis, TRALI, TACO and transmission of other infections remain possible. Thaw at 30–37 °C and transfuse promptly; do not refreeze.
Storage and supply
Store frozen at –25 °C or colder; after thawing, transfuse as soon as possible within the limit set by the blood service.
