What it is and how it works

Calcium gluconate provides ionised calcium, which is essential for neuromuscular excitability, cardiac conduction and contractility, and coagulation. In hyperkalaemia, calcium antagonises the membrane effects of potassium on the myocardium without lowering serum potassium.

Indications

Acute symptomatic hypocalcaemia (e.g. tetany); cardioprotection in severe hyperkalaemia; hypermagnesaemia and magnesium toxicity; calcium-channel blocker overdose (adjunct).

Dosage and administration (summary)

Adults: 10–20 mL of 10% solution (approximately 2.25–4.5 mmol calcium) by slow intravenous injection over at least 5–10 minutes with ECG monitoring, repeated as necessary or followed by continuous infusion according to serum calcium.

Contraindications

Hypercalcaemia; hypercalciuria; severe renal impairment with calcium stones; concomitant intravenous ceftriaxone in neonates; digitalis toxicity (relative).

Warnings and precautions

Rapid injection may cause bradycardia, arrhythmias, vasodilatation and hypotension; use with extreme caution in patients taking cardiac glycosides. Extravasation can cause tissue necrosis and calcinosis. Do not mix with bicarbonate, phosphate or sulfate solutions (precipitation); products in glass containers are not recommended for repeated or prolonged use in children or patients with renal impairment because of aluminium exposure.

Pregnancy

May be used in pregnancy when indicated.

Storage and supply

Store below 25 °C; do not refrigerate or freeze; do not use if crystals are present.
