List code

Pharmacologic class
Intravenous carbohydrate–alcohol solution (ethanol in glucose)

What it is and how it works

A solution of ethanol (typically 5%) in glucose (dextrose) for intravenous infusion. Ethanol is a competitive substrate for alcohol dehydrogenase, preventing formation of toxic metabolites of methanol and ethylene glycol, and also supplies calories; glucose provides additional energy and free water.

Indications

Antidote in methanol or ethylene glycol poisoning when fomepizole is unavailable; historically as a source of calories and for sedation in specialised settings.

Dosage and administration (summary)

Dosing is individualised according to protocol, aiming at a target blood ethanol concentration (around 1–1.5 g/L in toxic alcohol poisoning) with frequent blood ethanol and glucose monitoring; dose requirements increase during haemodialysis.

Contraindications

Hypersensitivity to the components, epilepsy, coma due to other causes, urinary tract infection, and concurrent disulfiram or metronidazole; caution in hepatic disease.

Warnings and precautions

Causes CNS depression, hypoglycaemia (especially in children and malnourished patients), hyponatraemia and fluid overload; vital signs, glucose and electrolytes must be monitored. Thrombophlebitis can occur with peripheral infusion. Interacts with other CNS depressants.

Pregnancy

Ethanol crosses the placenta; use only when the benefit in poisoning outweighs the risk.

Storage and supply

Store below 25 °C; do not freeze.
