- Pharmacologic class
- Colloid plasma volume substitute; modified fluid gelatin (succinylated gelatin or polygeline)
What it is and how it works
Modified gelatin solutions contain polypeptides derived from hydrolysed bovine collagen, chemically modified (e.g. succinylated) to increase molecular size and retention. They exert colloid oncotic pressure, expanding intravascular volume roughly in proportion to the volume infused, and are eliminated mainly by the kidneys.
Indications
Colloid volume replacement in hypovolaemia due to blood or plasma loss (e.g. haemorrhage, trauma, surgery, burns), prevention of hypotension (e.g. with epidural or spinal anaesthesia) and priming of extracorporeal circuits.
Dosage and administration (summary)
By intravenous infusion; volume and rate are individualised according to the patient’s haemodynamic status, blood loss and haematocrit, under clinical monitoring.
Contraindications
Hypersensitivity to gelatin or excipients, hypervolaemia, hyperhydration, severe heart failure, and severe coagulation disorders.
Warnings and precautions
Anaphylactoid and anaphylactic reactions may occur, so the first part of the infusion should be given slowly with monitoring and resuscitation facilities available. Large volumes cause haemodilution of red cells, plasma proteins and clotting factors; caution in cardiac or renal impairment, and calcium-containing preparations should not be infused through the same line as citrated blood.
Pregnancy
Use only if clearly needed, as hypersensitivity reactions could harm the fetus.
Storage and supply
Store below 25 °C; do not freeze.
