What it is and how it works

Maprotiline is a tetracyclic antidepressant that potently and selectively inhibits noradrenaline reuptake at presynaptic nerve terminals, with little effect on serotonin reuptake. It also has antihistaminic and moderate antimuscarinic and alpha-adrenoceptor blocking properties, contributing to sedation and adverse effects.

Indications

Depressive illness, particularly where sedation is required.

Dosage and administration (summary)

Adults: initially 25–75 mg daily orally in divided doses or as a single dose at night, adjusted according to response, usually to a maximum of 150 mg daily; lower doses in the elderly.

Contraindications

Recent myocardial infarction, heart block or arrhythmias; history of epilepsy or lowered seizure threshold; use with or within 14 days of an MAO inhibitor; mania; severe liver disease; angle-closure glaucoma and urinary retention.

Warnings and precautions

Risk of seizures is dose-related and higher than with many tricyclic antidepressants; avoid rapid dose escalation and high doses. Increased risk of suicidal thoughts and behaviour, especially in young adults, at the start of treatment. Antimuscarinic effects, orthostatic hypotension, QT prolongation and sedation may occur; caution in cardiovascular disease, prostatic hypertrophy and in the elderly; dangerous in overdose.

Pregnancy

Use only if clearly needed; neonatal withdrawal effects may occur after use in late pregnancy.

Storage and supply

Store below 25–30 °C, protected from moisture.
