What it is and how it works
A sterile solution of electrolytes (sodium, calcium, magnesium, chloride), a buffer (lactate and/or bicarbonate) and glucose as osmotic agent, instilled into the peritoneal cavity. Solutes such as urea, creatinine and potassium diffuse from blood across the peritoneal membrane into the dialysate, while the glucose-generated osmotic gradient removes water (ultrafiltration); higher glucose concentrations produce greater ultrafiltration. This listing (II) is one strength in a series of standard glucose-based solutions that differ mainly in glucose concentration; the exact composition is given in the product information.
Indications
Acute kidney injury and end-stage chronic kidney disease requiring dialysis, by continuous ambulatory (CAPD) or automated (APD) peritoneal dialysis; also used for certain fluid overload, electrolyte disturbances and poisonings with dialysable substances.
Dosage and administration (summary)
Dosing is individualised by the nephrologist: the fill volume, number of exchanges, dwell times and choice of glucose strength depend on the patient’s size, peritoneal membrane characteristics, residual renal function and ultrafiltration needs.
Contraindications
Recent abdominal surgery or injury, extensive abdominal adhesions or fibrosis, abdominal wall infections, hernias or conditions preventing effective dialysis, inflammatory bowel disease or diverticulitis, and severe respiratory insufficiency; lactic acidosis for lactate-buffered solutions.
Warnings and precautions
Strict aseptic technique is required to prevent peritonitis; cloudy effluent or abdominal pain should be investigated promptly. Monitor fluid balance, body weight, serum electrolytes, acid-base status and blood glucose, since glucose absorption may cause hyperglycaemia, weight gain and dyslipidaemia; high-glucose solutions should be used only as needed. Warm the bag to body temperature with dry heat only and do not use if the solution is cloudy or the container is damaged.
Pregnancy
May be used in pregnancy when dialysis is required, under specialist supervision.
Storage and supply
Store below 25–30 °C; do not freeze; keep in the overpouch until use.
