- List code

- Pharmacologic class
- Uricosuric agent
What it is and how it works
Probenecid inhibits renal tubular reabsorption of uric acid, increasing its urinary excretion and lowering serum urate. It also inhibits tubular secretion of penicillins and cephalosporins, raising and prolonging their plasma concentrations.
Indications
Long-term management of hyperuricaemia in chronic gout; adjunct to beta-lactam antibiotics to prolong plasma concentrations; reduction of nephrotoxicity with cidofovir.
Dosage and administration (summary)
For gout, taken orally starting at 250 mg twice daily for one week, then 500 mg twice daily, adjusted to serum urate; adequate fluid intake is required.
Contraindications
Acute gout attack (do not start during an attack), history of uric acid kidney stones, blood dyscrasias, children under 2 years and concomitant salicylate therapy.
Warnings and precautions
Maintain high fluid intake and consider urine alkalinisation to reduce stone risk; less effective in renal impairment. It interacts with many drugs (e.g. methotrexate, NSAIDs, zidovudine) by reducing their renal clearance.
Storage and supply
Store below 25 °C.
