Pharmacologic class
Parenteral nutrition (amino acid, glucose and lipid emulsion admixtures, often multi-chamber bags)

What it is and how it works

Total parenteral nutrition supplies the body’s requirements for protein (as amino acids), energy (as glucose and lipid emulsion), essential fatty acids and, with additives, electrolytes, vitamins and trace elements directly into the venous circulation, bypassing the gastrointestinal tract. Multi-chamber bags keep components separate until activated immediately before use.

Indications

Nutritional support in patients in whom oral or enteral nutrition is impossible, insufficient or contraindicated, e.g. intestinal failure, severe malabsorption, prolonged ileus, major gastrointestinal surgery or high-output fistulae.

Dosage and administration (summary)

Composition, volume and infusion rate are individualised by the nutrition team according to the patient’s weight, clinical condition, fluid and metabolic status; peripheral or central venous administration depends on osmolarity.

Contraindications

Hypersensitivity to egg, soya, fish or peanut proteins (depending on the lipid source) or other components; severe hyperlipidaemia; uncorrected severe metabolic disorders (e.g. metabolic acidosis, hyperglycaemia unresponsive to insulin, hyperosmolar states); congenital abnormalities of amino acid metabolism; unstable circulatory conditions such as shock.

Warnings and precautions

Monitor fluid and electrolyte balance, blood glucose, serum triglycerides, liver function and acid–base status regularly. Start cautiously in malnourished patients to avoid refeeding syndrome. Catheter-related infection, thrombosis and hepatobiliary complications may occur with long-term use; strict aseptic technique is essential.

Pregnancy

May be used in pregnancy when parenteral nutrition is clinically required.

Storage and supply

Store below 25 °C, do not freeze, and keep in the overwrap until use; after mixing chambers or adding additives, use promptly or as directed by the manufacturer.
