Ipratropium Bromide / Fenoterol Hydrobromide

Aerosol, Metered · Respiratory · 20 µg/50 µg/1 dose · 200 dose

List code
17611
Pharmacologic class
Bronchodilator combination; short-acting antimuscarinic plus short-acting beta2-agonist

What it is and how it works

Ipratropium bromide is an antimuscarinic that blocks acetylcholine-induced bronchoconstriction mediated by vagal pathways. Fenoterol hydrobromide stimulates beta2-adrenoceptors, relaxing bronchial smooth muscle. The two mechanisms are complementary, giving a greater bronchodilator effect than either alone.

Indications

Prevention and treatment of reversible bronchospasm associated with chronic obstructive pulmonary disease and asthma.

Dosage and administration (summary)

By metered-dose inhalation, usually 1–2 puffs (ipratropium 20 micrograms/fenoterol 50 micrograms per puff) up to 3–4 times daily, with up to 8 puffs in 24 hours; acute bronchospasm may require additional puffs as directed.

Contraindications

Hypertrophic obstructive cardiomyopathy, tachyarrhythmia, and hypersensitivity to fenoterol, ipratropium or atropine-like substances.

Warnings and precautions

Caution in uncontrolled diabetes, recent myocardial infarction, severe cardiac or vascular disease, hyperthyroidism, phaeochromocytoma, narrow-angle glaucoma (avoid contact with eyes) and urinary outflow obstruction. Beta2-agonists can cause serious hypokalaemia, tachycardia and paradoxical bronchospasm; increasing use indicates worsening disease requiring review, and asthma patients should also receive anti-inflammatory therapy.

Pregnancy

Use with caution, particularly in the first trimester and near term, as fenoterol may inhibit uterine contractions.

Storage and supply

Store below 25 °C; protect from heat and direct sunlight; do not puncture or burn the canister even when empty.
