- Pharmacologic class
- Radiopharmaceutical (investigational): lutetium-177-labelled fibroblast activation protein (FAP)-targeting peptide
What it is and how it works
Lu-177 FAP-2286 consists of a cyclic peptide that binds with high affinity to fibroblast activation protein, which is highly expressed on cancer-associated fibroblasts in the stroma of many solid tumours, linked via a chelator to lutetium-177. Lutetium-177 emits beta particles that deliver localised cytotoxic radiation to the tumour microenvironment, and gamma emissions permit post-treatment imaging.
Indications
Investigational targeted radioligand therapy for advanced FAP-expressing solid tumours, used within clinical trials or compassionate-use programmes after confirmation of FAP uptake by PET imaging; not an established licensed indication.
Dosage and administration (summary)
Administered by intravenous infusion in a nuclear medicine facility; activity and number of cycles are determined according to the study or treatment protocol.
Warnings and precautions
Potential adverse effects include myelosuppression, renal toxicity, fatigue and nausea; blood counts and renal function should be monitored. Radiation-protection precautions are required for patients, carers and staff.
Pregnancy
Contraindicated in pregnancy, as with other therapeutic radiopharmaceuticals.
Storage and supply
Store in lead shielding according to the manufacturer’s instructions and radiation-protection regulations; short shelf life due to radioactive decay.
