What it is and how it works
Cryoprecipitate is the cold-insoluble fraction obtained by controlled thawing of fresh frozen plasma. It is rich in fibrinogen, factor VIII, von Willebrand factor, factor XIII and fibronectin, and is used mainly to restore fibrinogen levels.
Indications
Hypofibrinogenaemia or dysfibrinogenaemia with bleeding or before invasive procedures, including massive haemorrhage, obstetric haemorrhage and disseminated intravascular coagulation; uraemic bleeding in some settings; factor XIII deficiency, haemophilia A or von Willebrand disease only when specific virally inactivated concentrates are unavailable.
Dosage and administration (summary)
Dosing is individualised according to fibrinogen level, clinical situation and transfusion protocol; in adults a typical dose is about 10 single-donor units (or two adult pools), guided by repeat fibrinogen measurement.
Contraindications
Not indicated when specific factor concentrates (e.g. fibrinogen, factor VIII, von Willebrand factor) are available and appropriate; IgA deficiency with anti-IgA antibodies requires special consideration.
Warnings and precautions
Units are not virally inactivated and carry a risk of transfusion-transmitted infection. Allergic reactions, transfusion-related acute lung injury and circulatory overload may occur; ABO-compatible units are preferred, especially in children. Thrombosis risk with large doses of fibrinogen in hypercoagulable states.
Pregnancy
May be used in pregnancy and obstetric haemorrhage when indicated.
Storage and supply
Store frozen at −25 °C or colder; after thawing keep at room temperature (20–24 °C) and use within 4 hours; do not refreeze or refrigerate after thawing.
